Mesothelioma Staging

Once an individual is diagnosed with mesothelioma, doctors must work to stage the disease. This is a crucial step in planning a treatment regimen and developing a prognosis. Staging refers to the system used by physicians to assess various aspects of the malignancy. When it comes to any of the four types of mesothelioma – pleural, peritoneal, pericardial, or testicular – most doctors around the world use a standard measurement for staging known as the TNM system. This universally used system helps to make assessments and treatment plans that are consistent with new trends in medications and procedures.

The TNM, which also goes by the name of IMIG Staging System, assess three issues that all influence treatment and prognosis factors.  T – The T represents the size and location of the tumor. Both of these factors are very significant when it comes to choosing a treatment plan.  N – The N represents node and it refers to the extent that the mesothelioma has or has not invaded the lymph nodes. This is also key in selecting an appropriate route for treatment.  M – The M represents metastasize and it reflects how much the mesothelioma tumor has spread to other areas, either in the same region or in other parts of the body entirely.

Once doctors make the assessments for each of these individually, they can combine the factors and have a comprehensive outlook for the disease. This outlook will then help determine which treatment type will be pursued.

Treatments Based on Staging

Surgery is most often the first procedure attempted for treatment purposes, even for all of the four different types of mesothelioma. While pleural mesothelioma tends to have the greatest success rate with surgery, the staging process can sometimes identify situations where surgery is appropriate for one of the other three mesothelioma types. Sometimes this means that staging determines that only a simple surgical procedure to drain fluid should be done that will help alleviate pain, and other times it helps doctors determine that the removal of an entire lung is warranted in patients with pleural mesothelioma.

If there is concern that the cancer might have either left a few cells behind after a surgery or be metastasizing, chemotherapy can be used to slow this growth. However, this is typically not a course of action taken for testicular or pericardial mesotheliomas. Radiation is also sometimes chosen based on the staging results, but usually in combination with a surgical procedure, and rarely (if at all) on pericardial or peritoneal mesotheliomas.
Because the latency period is so elongated in mesothelioma, detection and diagnosis is sometimes slowed. Once a diagnosis is confirmed, however, staging should be done as soon as possible so that there is time to make informed decisions regarding potential treatment plans.
