Staging and Treatment Options for Mesothelioma

The mesothelium is the name given to the protective membrane that surrounds different organs in the body. This membrane must provide lubrication so that the organ can move internally as needed. When a patient is diagnosed with malignant mesothelioma it means that on one of these membranes a cancerous growth exists, affecting the ability of the organ to function. The four types of mesothelioma are named for the part of the body they affect:

•    Pleural – the lungs
•    Peritoneal – the abdomen
•    Pericardial – the heart
•    Testicular – the testes

Staging Mesothelioma

When mesothelioma is diagnosed, physicians take into account several factors that help to apply a staging system to the diagnosis. This will then in turn help determine which treatment options are most viable in each unique situation. A majority of doctors around the world who treat mesothelioma use what is known as the TNM system, making universal treatment decisions more effective. Even though this TNM staging system is mainly focused on the pleural mesothelioma type, the other types of the cancer can apply it as well. There simply aren’t currently enough diagnosed cases each year to encourage doctors to adopt a separate staging system for peritoneal, pericardial, or testicular mesothelioma.
Also known as the IMIG Staging System, the TNM system gets its name from the three separate components of the diagnosis.

T = tumor, and refers to the size and location of the tumor
N = node, and it refers to whether or not the lymph nodes have been affected by the cancer
M = metastasize, and it refers to whether or not the disease has spread to other parts of the body

All three of these components are then examined on individual levels. For example, doctors will look at the tumor and determine whether or not the growth is limited in size and the involvement of major tissues, or whether or not surgery is an option to remove part of the tumor.

When it comes to the lymph nodes, doctors will determine whether or not they are free of cancer cells, and which nodes precisely are affected, as the type of node can also affect the treatment options. Finally doctors will determine whether or not the mesothelioma has metastasized into other areas of the body, and to what extent.

Based on the total of these assertions, a doctor will assign one of the following stages (although some just choose to stage I, II, III, or IV): Stages I, IA, IB, II, III, or IV. These detailed staging techniques can then help doctors formulate a treatment plan that is most appropriate to the type and severity of mesothelioma.
